Southwark Walking Away From Diabetes

Southwark’s Walking Away From Diabetes service has been designed to help reduce the incidence of type 2 diabetes in the borough among those in the borough who have high blood sugar but who do not suffer from the condition. This will be achieved through a variety of methods, while the programme also includes several secondary aims.

What Does the Walking Away From Diabetes Programme Involve?

The Walking Away From Diabetes programme in Southwark involves each service user attending a half day/three-hour structured self-management education course. This involves teaching the participants how to help avoid contracting the condition and how to reduce their blood sugar levels.

These sessions will cover how the service users themselves are feeling about the issues facing them and helping them to understand more about type 2 diabetes, its effects and what high blood glucose levels means. Service users will also learn about how being at risk of contracting the condition can effect long term health and understanding what factors contribute to them being at risk. Lastly, participants will learn about how to reduce the risk of contracting type 2 diabetes through increasing physical activity and eating more healthily, while we also help plan for the future.

Three months after this, there’s a one-hour follow up workshop or telephone evaluation to see how the service user is progressing, followed by a further telephone evaluation three months later. These can be arranged at the service user’s convenience, both during the week and at the weekend. The programme is delivered by two fully-qualified trainers and a family member/friend or carer is welcome to accompany the service user.

Who is the Service Available to?

The Walking Away From Diabetes programme in Southwark is available to all patients that are over the age of 18, identified as pre-diabetic, have an HbA1c result of between 6.0 and 6.4% and are registered at a GP practice within the borough. This is regardless of whether they have been through a health check or are registered at a practice that offers health checks.
