An exciting opportunity is available for an experienced, enthusiastic, dynamic and motivated Nutritionist with a passion and keen interest in weight management. This role will contribute to the development, provision and coordination of the Child Weight Management Service in Hackney. This community-based, multi component service offers a personalised care package for children from 5 – 25 years of age, with a focus on those overweight or obese, and those who are at high risk of obesity. This service is delivered by an expert multi-disciplinary team, providing advice on healthy eating and physical activity.

Playing a pivotal role within this service, you will be responsible for providing high quality, nutrition advice and support, including NICE Compliant Children’s Weight Management group delivery working within a multi-disciplinary specialist weight management team. You will provide expert nutrition advice, underpinned by a sound knowledge of theoretical principles of behaviour change, to support children and their families, in the London Borough of Hackney to make informed decisions in relation to their weight.

  • Providing high quality physical activity advice within a community-based, weight management service.
  • Delivering one to one and group interventions for children who are overweight or obese, with potential co-morbidities / complex needs.
  • Working as part of a multidisciplinary team.

This role will contribute to the development, provision and coordination of an integrated Lifestyle Service delivered across Cambridgeshire. This community-based, multi component service brings together a range of different programmes under one Countywide Lifestyle Service, centred on supporting residents to lead healthier lives.

Playing a pivotal role within this service, you will be responsible for providing high quality patient-centred, timely and cost-effective care within Tier 2 services working within a multi-disciplinary specialist weight management team. Providing expert nutrition advice, underpinned by a sound knowledge of theoretical principles of behaviour change, you will support children, young people and adults across Cambridgeshire to make informed decisions in relation to their weight, contributing towards a healthier lifestyle.

  • Delivering high standard nutritional care and advice within a community-based, weight management service.
  • Delivering one to one and group interventions, for adults and children who are obese, with potential co-morbidities / complex needs.

Playing a pivotal role within this holistic service, you will be part of a multi-disciplinary team in locations across Sandwell, responsible for the front line face to face delivery of services to help improve the health and wellbeing of local communities at clinics and local events.

The service offers support for specified adults through preventative screening to identify and manage those at risk of a range of vascular diseases such as heart disease, stroke, diabetes, dementia and kidney disease and to offer them appropriate advice, referral or follow up.

  • Delivering NHS Health Checks within the local community and in GP Surgeries
  • Creative and pro-active with strong planning, time management, empathy and interpersonal skills.

Stop Smoking Practitioner – Sandwell

The stop smoking practitioner is responsible for supporting and facilitating smoking cessation programmes within the stop smoking service in Sandwell. You will be providing high quality behavioural support and pharmacological advice to Sandwell residents who wish to stop smoking. Whilst improving the accessibility of the Stop Smoking Service through the pro-active promotion of the smoking cessation programme and referral pathways amongst primary and secondary care services, other key health care providers, allied professionals, community development initiatives, local agencies and the general public.

If you are enthusiastic, motivated and looking to make a difference then this is the role for you.

We would like to hear from you, if you: 

  • Have strong interpersonal and communication skills.
  • Possess a minimum of 1 year experience working in a community based health improvement service.
  • Are experienced in using behaviour change models within a work context.
  • Display strong knowledge of the harmful effects of smoking, smoking as an addiction behaviour, withdrawal and relapse
  • Are flexible with good time management and planning skills
  • Are interested in motivating and supporting both individuals and groups in community based settings.
  • Can raise public awareness and provision of consistent healthy lifestyle messaging, on issues relevant to the health of the community and in a multi-cultural setting.
  • Have experience in building strong links and relationships with a diverse range of groups, organisations and agencies
  • Are currently registered with the NCSCT as a stop smoking practitioner (or willing to work towards this qualification)
