Social Prescribing Project Management – Southend-on-Sea

2 – 3 day per week. Salary Competitive, dependent upon experience

The role will develop the implementation of the agreed social prescribing model in Southend-on-Sea to enable the system to further improve the health and wellbeing of an individual through ensuring that the intervention best meets their tailored goals. The role will ensure that there is effective interface and communication between the Primary Care Networks (PCNs), SAVS, SBC and Everyone Health in the effective implementation of social prescribing.

Responsibility to;

  • Lead on the planning and delivery of the social prescribing programme, working with PCNs, SAVS and key local partners.
  • Lead on engagement for the development of a digital referral management tool for the scheme.
  • Support work with SAVS development officers to ensure that all community providers’ individuals are referred into have quality assurance.
  • Effectively engage all partners in health and voluntary services with the project and ensure they are briefed on the project aims and objectives.
  • Manage relationships and agreements with commissioners and providers e.g. GPs, CCGs, VCSs, funders, commissioners and communities.
  • Develop a simple process to enable the appropriate referrals using social prescriptions effectively.
  • Ensure that there are joint working arrangements in place with other local prescribing projects to ensure the success of the social prescription project.
  • Set up a process to identify, report and monitor any safeguarding or vulnerable adult risks and concerns.

Everyone Health are currently seeking advisors to join our growing Health Services team in our Clinical Contact Centre. Our single and multi-component services provide public health services including prevention and early intervention services, specifically targeting those at high risk.

We require motivated, empathetic and flexible individuals who will play a critical role in virtually supporting patients as they move through the defined pathways of care. Utilising behaviour change techniques and SMART goal setting on a 1:1 telephone and e-basis you will ensure our patients achieve sustainable changes in their lives.

These vital roles provide coverage and support to all regional contracts across the UK, undertaking the virtual triage of patients.

Located in Hinckley, the Clinical Contact Centre operates on a shift pattern 6 days a week.

The successful candidate will:

  • Have experience of providing 1:1 support and triage to patients & clients, resulting in positive healthy lifestyle behaviours.
  • Ideally, have recognisable Health Trainer & Public Health Qualifications
  • Be emotionally sensitive and empathetic to members of the public who wish to improve their health
  • Possess knowledge of behaviour change and theoretical principles that support people when embarking on a behaviour change
  • Show knowledge of health inequalities and the barriers to health improvement
  • Demonstrate understanding of patient confidentiality, data protection and information governance
  • Be proficient in using software systems to document, collate and evidence work

An exciting opportunity is available for an experienced, enthusiastic, dynamic and motivated Nutritionist and Physical Activity practitioner with a passion and keen interest in weight management. This role will contribute to the development, provision and coordination of the Child Weight Management Service in Hackney. This community-based, multi component service offers a personalised care package for children from 5 – 25 years of age, with a focus on those overweight or obese, and those who are at high risk of obesity. This service is delivered by an expert multi-disciplinary team, providing advice on healthy eating and physical activity.

Playing a pivotal role within this service, you will be responsible for providing high quality, nutrition advice and support, including NICE Compliant Children’s Weight Management group delivery working within a multi-disciplinary specialist weight management team. You will provide expert nutrition advice, underpinned by a sound knowledge of theoretical principles of behaviour change, to support children and their families, in the London Borough of Hackney to make informed decisions in relation to their weight.

We would like to hear from you if you are experienced in:

  • Providing high quality physical activity advice within a community-based, weight management service.
  • Delivering one to one and group interventions for children who are overweight or obese, with potential co-morbidities / complex needs.
  • Working as part of a multidisciplinary team.
  • And have current Association for Nutrition Registration

  • Level 2 Children’s Exercise Certificate (or equivalent)
